Assisted reproductive technologies (ART) are the methods of treatment, in which manipulations with the reproductive cells, some or all of the stages of preparation of reproductive cells, fertilization and embryo genesis processes before transferring the embryos into the uterus of the patient are carried out in vitro.
The history of the department of assisted reproductive technologies in Academician V.I. Gryshchenko Clinic for Reproductive Medicine extends back over 20 years, and it was this department that gave rise to the development of the Clinic and is associated for many patients with our previous name “Implant”.
Today the effectiveness of reproductive technologies is high enough, but unfortunately it is far from 100 percent. Our Clinic works with the effectiveness being comparable and sometimes even exceeding Central European and American standards.
Everything flows, everything changes. Changing are approaches to reproductive technologies as well, perception of their objectives and the needs of our patients. If earlier the primary objective was to achieve the pregnancy at any cost, now we think above all about the health of our patients and their future children.
If we try to formulate very concisely the basic objective of reproductive technologies, then in our understanding it will be as follows:
“Giving birth to a healthy baby, which will grow a healthy adult, in the absence of danger for health and life, with maximum comfort, without significant changes in life style in connection to the treatment, with minimal expense, as soon as practically possible."
It is not always possible to solve all these problems on the way to achieving one overarching objective. But we should strive for this. Today, in Academician V.I. Gryshchenko Clinic for Reproductive Medicine, Department of ART both for equipment and professional experience corresponds to the highest world standards.
We have analyzed the results of 1264 IVF cycles performed in Academician V.I. Gryshchenko Clinic for Reproductive Medicine , for the period from January 01 to December 31, 2014.
The basic criterion of effectiveness of the Clinic should be considered to be the frequency of implantation. This figure is calculated by the method of determining what percentage of transferred embryos yielded results in the form of progressive pregnancy with visualization of the fetal cardiac activity. For the above-mentioned period at the Clinic, of 2333 transferred embryos, 792 have been implanted. Thus, the frequency of implantation amounted to 33.9%.
It is known that the IVF effectiveness is greatly reduced with age. The main cause of the deterioration of results is an increase in the number of chromosomal abnormalities of embryos, resulting in a decrease in the frequency of implantation due to the embryonic factor. To exclude its effect, we have calculated the rate of implantation in young female patients (less than 35 years old at the time of embryo transfer), which, in fact, are a selective marker subgroup, in which usually the quality of the Clinic’s performance is estimated. In this group, of 1654 transferred embryos, 627 have been implanted – that is, the frequency of implantation was 37.9%.
For comparison, according to the data of Society for Assisted Reproductive Technology (SART, USA, http://www.sart.org), the frequency of implantation in the same selective group in the United States in 2012 was 37.5%.
To exclude additional embryonic factors that can affect implantation, we have separately calculated the frequency of implantation in oocyte donation cycles. This figure for Academician V.I. Gryshchenko Clinic amounted to 42.4% (84 of 196 embryos have been implanted).
These results show that operating results of Academician V.I. Gryshchenko Clinic for Reproductive Medicine are high, stable and comparable with the operating results of the best American and European clinics.
